Title: Sistemas adesivos aplicados ao dente utilizando corrente elétrica

Abstract: Resumo: Objetivos: O presente trabalho foi dividido em dois estudos, inicialmente com realizacao de uma revisao sistematizada sobre a aplicacao de sistemas adesivos a dentina sob corrente eletrica (capitulo A). Posteriormente foi realizada analise da resistencia da uniao (RU) e da camada hibrida dentina - resina composta com diferentes sistemas adesivos aplicados sob corrente eletrica e fotoativados por diferentes irradiancias (capitulo B). Metodologia: No capitulo A foi realizada tres estrategias de busca, sem limite de ano de publicacao, para uma revisao sistematizada de publicacoes vinculadas ao uso de um dispositivo de corrente eletrica na inducao de sistemas adesivos ao substrato dentinario. Para o capitulo B, molares humanos foram divididos em grupos de acordo com o sistema adesivo (Adper Single Bond 2-SB, 3M ESPE; Clearfil SE Bond-CL, Kuraray; e Single Bond Universal-SU, 3M ESPE), corrente eletrica (0-controle e 50ƒÊA) e fotopolimerizador (monowave - Radii cal, SDI - e polywave - VALO Cordless, Ultradent). Os grupos (n=6) foram submetidos ao ensaio da RU a microtracao (MPa). Os padroes de fratura foram classificados em falhas adesiva, coesiva em dentina/resina e mista (lupa estereomicroscopica 50x e microscopia eletronica de varredura; JSM-5600LV, JEOL). A interface adesiva foi analisada em microscopia confocal de varredura a laser (Leica SP5, Leica Microsystems) (n=2). Os dados foram submetidos ao teste parametrico de Kolmogorov-Smirnov e a analise de variancia (ANOVA) 3 fatores (adesivo, aplicacao, irradiancia), sendo as medias analisadas pelo teste de Tukey post-hoc (ƒ¿=0,05). Resultados: Sobre o tema pesquisado para revisao sistematizada (capitulo A) foram encontrados onze trabalhos, publicados nos anos de 2006 a 2017. Todos encontraram efeitos positivos na tecnica de aplicacao de sistemas adesivos com corrente eletrica, com aumento da RU dente-resina composta. No capitulo B, apenas o adesivo SU fotoativado com fotopolimerizador monowave apresentou melhora nos valores de RU com a utilizacao da corrente eletrica. As falhas do tipo adesiva foram predominantes em todos os grupos. A microscopia confocal mostrou que o uso da corrente eletrica formou tags resinosos mais longos, principalmente com uso do fotopolimerizador polywave para SB e SU. Consideracoes finais: Poucos os estudos avaliam a aplicacao da corrente eletrica na inducao de materiais resinosos no substrato dentinario. Existe uma falta na padronizacao da intensidade da corrente aplicada. Os dados do estudo laboratorial (capitulo B) corroboram parcialmente com os resultados positivos do uso do dispositivo de corrente eletrica com sistemas adesivos, pois outros fatores, como o tipo de sistema adesivo empregado e irradiancia utilizada na fotoativacao, interagem com a corrente eletrica e influenciam nos valores de adesao.
Abstract: Objectives: This research was divided into two studies; initially, a systematic review regarding the application of adhesive systems onto the dentin under electric current (chapter A), then, a dentin-resin composite tensile bond strength (TBS) analysis with different adhesive systems applied under electric current and photoactivated with different irradiance (chapter B). Method: In chapter A, three research strategies were applied, with no limitation regarding the year of publication, for a systematic review of publications concerning the use of an electric current device during the application of adhesive systems to the dentin substrate. For chapter B, human molars were distributed into groups according to the adhesive system to be used (Adper Single Bond 2 - SB, 3M ESPE; Clearfil SE Bond - CL, Kuraray; and Single Bond Universal - SU, 3M ESPE), electric current (0-control and 50ìA) and light curing units (monowave-819mW/cm2; Radii Cal, SDI, and polywave-1283mW/cm2; VALO Cordless, Ultradent). The samples were submitted to tensile bond strength (MPa) testing. The fracture patterns were classified according to their adhesive failure: cohesive on dentin/resin and mixed (50x-stereomicroscopic lens, and scanning electronic electroscopy (SEM); JSM-5600LV, JEOL). The adhesive interface was analysed using a Confocal Laser Scanning Microscope (Leica SP5, Leica Microsystems). The data were submitted to Kolmorogov-Smirnov parametric testing and to the analysis of variance (3-way-ANOVA, adhesive, application, and irradiance), with mean values analysed with the post-hoc Tukey test (á=0,05). Results: Regarding the systematic review (chapter A), eleven studies published between 2006 and 2017 were found. All presented positive effects concerning the application of adhesive systems associated with electric current application, with results showing an increase in the dentin-composite resin tensile bond strength. As for chapter B, only the photoactivated SU adhesive with monowave presented improvement in the bond strength values with the addition of the electric current. The adhesive-type failures were predominant in all groups. The confocal laser scanning microscopy showed that the use of electric current formed longer resinous tags for SB and SU, especially under polywave. Final considerations: Few studies have evaluated the application of electric current in the induction of resinous materials in the dentin substrate. There is lack of standardisation in the applied current intensity. The laboratorial study data (chapter B) partially corroborate with the positive results involving the use of the electric current device with adhesive systems, as other factures, such as the type of applied adhesive system and the irradiance applied during photoactivation, interact with the electric current and influence the bonding values. Future studies evaluating the long-term effects of electric-current induction of adhesive systems and other physical-chemical properties are required for better understanding of this restorative technique.
