Abordagens do paciente com síndrome de down para tratamento odontológico : revisão de literatura

Abstract

Resumo: A Síndrome de Down (SD) é causada por uma alteração genética no par de cromossomos 21, há um cromossomo extra, assim chamada de trissomia 21. Essa alteração, durante a divisão celular, acarreta características bem definidas, distúrbios mentais e sistêmicos, e alterações bucais. A literatura cita como alterações bucais da síndrome, macroglossia, maloclusão, erupção dentária tardia, língua fissurada, além do alto índice de doença periodontal, cárie e bruxismo, e entre os problemas sistêmicos, cardiopatias e deficiência no sistema imunológico. Necessitando esses pacientes de acompanhamento multidisciplinar. O objetivo desse trabalho é apresentar e informar aos cirurgiões dentistas as características e cuidados a serem tomados para o bom planejamento e atendimento odontológico. A revisão de literatura de 2008 a 2020 foi realizada nas plataformas Google acadêmico, Pubmed e SciELO. Concluindo que, o atendimento precoce e a participação dos responsáveis, motivando e auxiliando esses pacientes a manterem uma boa higiene bucal, é fundamental; quando tratados com carinho, respeito e amor, são pessoas carinhosas, e colaborativas, permitindo assim serem tratados no consultório odontológico, evitando o uso de anestesia geral, concorrendo para a promoção de sua saúde bucal e geral. É necessário que os cirurgiões dentistas tenham conhecimento das características físicas e das alterações bucais dos sindrômicos. Entretanto não há um protocolo de atendimento para pacientes com SD

Abstract: Down Syndrome (DS) is caused by a genetic alteration in the pair of chromosomes 21, there is an extra chromosome, so called trisomy 21. This alteration, during cell division, causes well-defined characteristics, mental and systemic disorders, and alterations mouthpieces. Literature mentions as oral alterations of the syndrome, macroglossia, malocclusion, late tooth eruption, fissured tongue, in addition to the high rate of periodontal disease, caries and bruxism, and among systemic problems, heart disease and deficiency in the immune system. These patients need multidisciplinary follow-up. The objective of this work is to present and inform dental surgeons about the characteristics and care to be taken for good planning and dental care. The literature review from 2008 to 2020 was carried out on the academic Google, Pubmed and SciELO platforms. Concluding that early care and the participation of those responsible, motivating and helping these patients to maintain good oral hygiene, is essential; when treated with affection, respect and love, they are caring and collaborative people, thus allowing them to be treated in the dental office, avoiding the use of general anesthesia, contributing to the promotion of their oral and general health. It is necessary that dental surgeons are aware of the physical characteristics and oral alterations of syndromics. However, there is no care protocol for patients with DS
