Alterações de manchas brancas no esmalte dentário : revisão de literatura

Abstract

Resumo: O esmalte é um tecido mineralizado, de origem ectodérmica, que reveste e protege o elemento dentário, podendo sofrer alterações em sua formação, o que promove manchamentos aparentes clinicamente. Esta revisão de literatura apresenta as características de cada uma das alterações de manchas brancas no esmalte dentário mais frequentes, cárie dentária, hipoplasia de esmalte, fluorose dental e hipomineralização molar incisivo, bem como o que traz a literatura como meios para a identificação e realização do diagnóstico diferencial. Justifica-se a importância para o profissional em conhecer e analisar as alterações que atinguem o tecido dentário, auxiliando na dificil tomada de decisão clínica, quanto ao diagnóstico diferencial e a indicação do tratamento mais adequado, visto que as alterações no esmalte,aqui estudadas, têm características clínicas semelhantes. O levantamento bibliográfico emvolveu publicações científicas nacionais e internacionais, a partir das bases de dados Google Acadêmico, eScielo, PubMed, Eric, no período entre 2012 e 2021. O que permitiu concluir que: as alterações do esmalte dentário caracterizadas por manchas brancas, são semelhantesclínicamente, o que dificulta seu diagnóstico diferencial; o diagnóstico diferencial para essas alterações, será possível mediante anamnese detalhada, o conhecimento das características e fatores etiológicos de cada uma delas; o diagnóstico correto faz-se necessário na busca por um tratamento eficaz para recompor a função e estética dos dentes, bem como a auto-estima dos pacientes.

Abstract: Enamel is a mineralized tissue, of ectodermal origin, which covers and protects the dental element, and may undergo changes in its formation, which promotes clinically apparent stains. This literature review presents the characteristics of each of the most frequent alterations of white spots on tooth enamel, tooth decay, enamel hypoplasia, dental fluorosis and molar - incisivorr hypomineralization, as well as what the literature presents as a means for identifying and performing the differential diagnosis. It justifies the importance for the professional to know and analyze the changes that affect the dental tissue, helping in the difficult clinical decision-making, regarding the differential diagnosis and the indication of the most appropriate treatment, since the changes in enamel, studied here, have similar clinical features. The bibliographic survey involved national and international scientific publications, from the databases Google Academic, eScielo, PubMed, Eric, in the period between 2012 and 2021. Which allowed us to conclude that: dental enamel alterations characterized by white spots are clinically similar , which makes its differential diagnosis difficult; the differential diagnosis for these alterations will be possible through detailed anamnesis, knowledge of the characteristics and etiological factors of each one of them; the correct diagnosis is necessary in the search for an effective treatment to restore the function and esthetics of teeth, as well as the patients' self-esteem.
